Results page - missing answers on required fields

Bug reporting steps: These were "required" fields, and it's not possible they didnt add the fields. Some of them are multiple choice. What could be the issue here? We see 5 different pieces of information missing here in the application What we need to do next: investigate how this scenario happens and what's the root cause It could either be results page data pull (that would be the easiest), or wrong validation or missing some data capture at the time of completing the form We need to make sure we're not losing any data
